Okay, so you know how Allison never told any of the girls her secrets? Well the big secret she was hiding was that she has a mental twin sister named Courtney. Courtney escapes from the mental hospital and pretends to be Allison. Then the real Allison is forced to go back to the mental hospital because they think it’s Courtney. In the flashbacks they show, it’s all Courtney but the girls don’t know this. In the first episode, when Courtney (pretending to be Allison) disappears, Allison kills her. 

I know this sounds confusing, but yeah.
